Dr. Anderson is an in-network provider for the following health insurance companies:

Since each insurance company has a variety of plans we recommend that you contact your insurance company directly to clarify the following:

• Do you have a deductible? If yes, what portion of the deductible is remaining?
• Are you responsible for a co-pay or co-insurance?
• Do you need a referral or prior-authorization in order to receive psychotherapy or psychological testing? Is psychological testing for AD/HD and learning disorders covered under your individual policy?

(Note: you may reference billing code 96101 for psychological testing.)
